GPs, pharmacists, hospitals and laboratories all work with your own medical records. With your permission, your healthcare provider may have the data relevant to them at:
- Spaarne Gasthuis (hospital)
- Atalmedial (the Haarlem laboratory)
- Streeklab Haarlem (microbiology laboratory)
This allows relevant data to be made available to your healthcare provider properly, securely and quickly. So your healthcare provider - GP, specialist and pharmacist - is better informed about your health. This benefits your treatment. For example, it may be important for your GP to be able to see results of blood tests requested by a specialist. How do you give permission?
This can be done in two ways:
- You give your consent preferably yourself, digitally via the secure part of the website www.mijndoc.nu. You will need your DigiD with SMS function to do this. You use your mobile or landline phone for this. With the SMS function via a landline phone, you will receive a spoken text message.
- You can also arrange your consent with your GP via a consent form available at your GP's practice (as long as coronagraphs apply, telephone contact first and do not come to the practice without an appointment!). You can also find the form opposite. You can download the form, print it out and hand it in at the health centre. Only with your consent can your healthcare provider have relevant data. You choose which groups of healthcare providers your permission applies to: GPs, specialists and pharmacists (and the staff and observers of these healthcare providers). You can withdraw your consent at any time via www.mijndoc.nu. Or through your healthcare provider. It is important that you make a conscious choice. You may need some more information to do so.
