Do you have a referral or application form from your doctor? Bring this with you to your appointment, along with your proof of identity.
Your doctor will give you a (digital) request form. With that form, you can have blood or urine taken from us. After the test, your doctor or specialist will receive the results. Your doctor will tell you when to expect the results.
