Heeft u een verwijzing of aanvraagformulier van uw arts? Neem dit mee naar uw afspraak, samen met uw legitimatiebewijs.
Uw arts geeft u een (digitaal) aanvraagformulier mee. Met dat formulier kunt u bij ons bloed laten afnemen of urine afgeven. Na het onderzoek ontvangt uw arts of specialist de uitslag. Wanneer u de uitslag kunt verwachten, spreekt u af met uw arts.

If you have any questions about your results, you should call the practice yourself. Add the ‘’Call for an appointment’’ button, note white letters should be used.
For less urgent complaints, you can send us an e-Consult. Our practice will then usually answer your query within 72. Since you do not call, this saves us incoming calls and helps us spread out crowds.
