This will be given to you or sent to us directly by your GP. For children under 16, a signed declaration of consent is required from the parents\/guardians with parental authority. During the intake interview, the connection between the circumstances and the complaints will be examined. In addition, the request for help, goals and expectations of the examination or treatment and the possibilities for this will be discussed.
