With professional expertise and the best facilities, the staff at the Health Centre are committed to providing you with the best possible service. Should you nevertheless be dissatisfied, we would like to hear about it

What do we do with your complaint?

The most important thing is to see with you whether the complaint can be resolved to your satisfaction. Another factor is that we use your complaint to improve our services if necessary. For the purpose of its complaints procedure, Schalkwijk Health Centre has established a procedure by which a complaint is handled. This procedure states, among other things, that a complaint must be dealt with in a maximum of four weeks

What if Schalkwijk Health Centre's complaints procedure does not provide relief?

It may happen that you are dissatisfied with the outcome of Health Centre Schalkwijk's handling of your complaint in which case you can submit your complaint to the complaints committee to which the care provider concerned is affiliated. Below you will find the corresponding addresses and websites for the various disciplines; on these sites you will also find all the information on how your complaint will be handled.

It is best to discuss your complaint with the person involved first. You tell them what you are dissatisfied about and the person involved will then have the opportunity to change certain things or explain the course of events to you. You may reach a solution in such a discussion. If the complaint is not resolved, you can submit the complaint to the Health Centre. You can also contact the Health Centre directly.

 You can file your complaint in different ways.

  • When making a complaint, you can use the online complaint form below.
  • Or you can download the complaint form, fill it in completely and send it to the health centre postal address below or you mail the complaint to klachten@gcschalkwijk.nl
  • You describe your complaint as best you can in an e-mail message
  • You come to report your complaint verbally. The complaints coordinator will then try to describe your complaint as best as possible using the complaint form and take it up.

It may be that you are not satisfied with the organisation, communication or the way you were treated in the health centre and you cannot work it out together? Then you can submit a complaint to the complaints committee to which the healthcare provider is affiliated.

Results

You can view your data, either through the GP or online. Online, you can view your data using your phone or computer, wherever you are. You log in to a secure website or app. You will see key information such as your diseases, your medications and results of tests.

If you have any questions about your results, you should call the practice yourself. Add the ‘’Call for an appointment’’ button, note white letters should be used.

E-Consult

For less urgent complaints, you can send us an e-Consult. Our practice will then usually answer your query within 72. Since you do not call, this saves us incoming calls and helps us spread out crowds.

Access to medical records

Via MyHealth.net it is possible to access your medical records. It is possible to gain insight into the items below:

  • Laboratory results
  • Physical examination
  • Access log
  • Report
  • Hypersensitivities and peculiarities
  • My documents
  • Current medication overview

Data exchange

Your (GP) doctor and pharmacies can connect their computers to the National Switch Point (LSP). Through the LSP, they can share your medical data. Another healthcare provider can view those medical details. The video below briefly explains how the LSP works when you come to the GP surgery.
